Position is located in beautiful Sauk City, Wisconsin.
Operating Hours are 8AM-4:30PM, Monday-Friday.
No scheduled weekends!
2.5 days/week (2 days week 1, 3 days week 2 of a 2 week pay period)
Flexibility to offer more hours, however, if taking more than 50% of hours, expected to take on manager role.
Rate: Approximately $65/hour
Current pharmacist to assist with onboarding.
Required Duties would include:
*Answering medication related questions from residents, residents' families and professional staff.
*Attending monthly Admission/Discharge Meeting, if scheduled that same day.
*Attending weekly Interdisciplinary Meeting, if scheduled that same day.
*Attending Quarterly Quality Assurance Meeting, if scheduled that same day.
*Attending Monthly Leadership Meeting, if scheduled that same day.
*Checking prescriptions for accuracy
*Completing 50% of monthly medication reviews each month.
*Completing new medication reviews, as necessary.
*Completing Change in Condition medication reviews, as necessary.
*Contacting Insurance Companies, as necessary.
*Checking prescriptions orders.
*Contacting prescriber for clarification/to make recommendations as needed.
*Discharge Medication Counseling
*Attain Functional knowledge of ECS/American Data eMAR software
*Attain Functional knowledge of QS1/PrimeCare/RedSail Pharmacy Software
*Triaging tasks and directing staff as needed to keep monthly cycle fills completed in a timely manner
*Taking on-call 50% of weekdays and weekends. Rotating Holidays/Holiday weekends annually. Trade days/weekends available.
Preferred Candidates will have:
*Experience billing and running revenue reports.
*Experience in Long Term Care Pharmacy setting
*Experience conducting Monthly Medication Reviews
*Familiarity with sections pertaining to Pharmacy Services in CMS's most recent State Operations Manual Appendix PP.
*Familiarity with applicable State rules and regulations pertaining Long Term Care Pharmacy practice.
*Hold Credential of Certified Geriatric Pharmacist, or the like.
*Willingness to assist formulating Policies and Procedures as needs arise.
However, the willingness to learn is an asset.
