Our 120-bed facility is nestled along the Wisconsin River, just minutes from Madison, which offers the scenic beauty and friendliness of the Sauk-Prairie community as well as the finest professional services available.

Established in 1971, Maplewood has a history of being responsive to the changing needs of our aged population. Over the years, we have enhanced our nursing services with respite and hospice care, Alzheimer's/dementia care, psychotherapy, complete rehabilitation services, recreational activities and an in-house pharmacy. In 1999, we opened Maplewood Village, a state of the art assisted living facility located right next door to the skilled nursing facility.

Caring for the whole person in terms of their physical, social, emotional and spiritual needs facilitates the recovery process, encouraging the residents on to an independent lifestyle is Maplewood's focus. This person-directed approach to healthcare emphasizes the individuality, dignity and self-worth of each resident, ensuring that Maplewood of Sauk Prairie will always be a loving and nurturing place to live.
